Travel Sanctioning Form - A completed Travel Sanction Form is required for all teams travelling outside of the province to participate in a Lacrosse tournament or other organized event. This form must be completed in full and sent to the Lacrosse NS office for approval. Failure to submit a travel sanctioning form will result in the inability to access LNSS travel insurance.
Sanctioning Form - A completed LNS Sanction Form is required for all teams, clubs, or leagues participating in an LNS-sanctioned activity/program or event.
Teams, Clubs, and Leagues must submit a Sanction Form for their activity/program/event to be officially sanctioned. Failure to submit a Sanctioning Form to the Lacrosse Nova Scotia office will result in the inability to access LNSS Insurance.
Certificate of Insurance Request Form - If the venue you are holding your event in requires a certificate of insurance, please fill out this form and email it to the Lacrosse NS ED. All LNS Sanctioned Programs, Activities, and Events require an insurance certificate.
LC Head Injury Report Form - Lacrosse Canada (LC) has implemented a Head Injury Incident Report Form to be used for reporting head injuries to LC directly.
LC Goalkeeper Equipment Exemption Form - The LC Goaltender Equipment Exemption form must be filled out and sent to LC for approval by all goalkeepers using equipment outside of legal specifications.
Exceptional Player Status Form - This form is intended for provincial player use only. If an athlete wishes to be considered for the provincial team at an age level above their current age level, please submit this form by the deadlines indicated.
Accident Claim Form - Use this form to make an injury claim.
LNS Insurance Forms - Please note that LNS insurance is a secondary accident and liability insurance that can only be used once your primary insurance is exhausted or if it doesn't cover the necessary treatment for a lacrosse-related injury or incident.
